Central America's regional club championship, launched in 2023, whose winner advances to the Concacaf Champions Cup.

The Concacaf Central American Cup is the regional club championship of Central America, launched in 2023 as part of Concacaf's overhaul of its competition structure to create a clearer qualification pathway into the Concacaf Champions Cup. Twenty clubs from the region's national federations enter, drawn into four groups of five teams using a seeded pot system, with each side playing the others in its group once. The top two clubs from each group advance to a straight knockout stage, played over two legs, that runs through to the final. The tournament's champion, along with several other high finishers, earns a place in the following year's Concacaf Champions Cup, putting Central American clubs in direct contention with the top sides from Mexico, the United States and the Caribbean. Costa Rica's LD Alajuelense has dominated the competition's short history, winning all three editions played so far, seeing off Nicaragua's Real Estelí in the first two finals and Guatemala's Xelajú MC on penalties in 2025.
